I understand that my initial session (60 mins) with Georgia must be scheduled within 3 months of booking. The second session (20 mins) will be scheduled 2-3 weeks after the first, and no more than 2 months after the first session. This package is not refundable. Sessions may be rescheduled up to one time, as long as more than 48 hours notice is provided.

I understand and agree that the information I provide in this form will be used to deliver Vocal SOS, and kept in accordance with the GL Voice Privacy Policy.
